How do I become a patient?

Just go to our Become a Patient page and download one of the new patient packets to complete and return.  Please fill out one packet per person completely and attach copy of your insurance card.  You can designate your preferred physician(s) on the packet.  Not all physicians are taking new patients at all times.  One of our staff will contact you within a few days of receiving your packet.  If you need help completing your packet or have additional questions, please call our office and one of our staff would be happy to assist you.

 

What insurances do you accept?

We accept most insurance plans.  Please call 812-524-3333 if you have questions about a specific plan?

What happens if I need to be in the hospital?

If you need to be hospitalized and are able to be cared for at Schneck Medical Center, Family Medical Center (FMC) has partnered with the Schneck Hospitalists to take care of you.  The Hospitalist team will communicate with with your primary care team at FMC to help facilitate your care.  Upon discharge, your hospital nurse will contact a member of the FMC team to update us with any changes to your medications and upcoming medical needs as you transition back to home.  We will arrange a hospital follow-up visit with your doctor within a week of your discharge to make sure this transition goes smoothly.

Please download and complete the appropriate new patient packet.  One of our staff will contact you within a few days of receiving your completed forms.   Please include a copy of your insurance card(s) with your packet.  You may bring your fully completed packet by the office, fax it to 812-524-3334, or use our upload form.
